The First Visit

    Regrettfully, until March 1, 2012, I am not scheduling new patient evaluations.  There are too many conflicting demands which prevent me from having sufficient time to see new patients.

    Additionally, I am no longer seeing new patients under the age of 13 years.

     The first visit to a physician can make many people anxious. Information on this website is designed to help alleviate some of that anxiety.

 
   The first visit lasts 90 minutes. There is a lot of information to cover. My goal is to understand the symptoms/problems that caused you to make the appointment.  I will review your prior treatments, your health and family history, and general information about you.  I will do my best to insure that I have a good understanding of you. We can't cover everything, but I want to make sure that we cover the more important things. I need your help to be able to do this.    I shall ask you to complete a fair amount of paperwork prior to the visit. The paperwork covers most of the topics I shall be asking you about. This will familiarize you with some of the things I will be asking about.      Most of the paperwork may be printed from my website. 

 

Click on the appropriate link below (New Patients < 18 or New Patients > 18)  Print the pages, fill them out, and bring them with you to the first appointment.  Also, print the New Patient Registration form.      

 

There is also an "electronic" form to fill out. This is done online.  It covers your behaviors and feelings. When I score the questionnaire, your answers are compared with a sample of "Average individuals (or parents)". This allows us to see how you (or your child) are doing compared to the Average Person. This questionnaire takes about 15-20 minutes to complete. You can start, stop, and resume at a later time. Your answers are secure and encrypted.       I will email you your password (your Access Key Code).  You will go to www.entry-online.org and enter your Access Key Code.  I will receive an email once you have finished the form. We shall review this form during our visit.         

 I will need to send you the Access Key Code.  This will come in an email (see Secure Email).              


Please call to confirm your first appointment.  Call the office (210 694 0229) 48 hours before your first visit to confirm that you are coming. If we don't hear from you, we shall assume that you are not coming and your appointment will be cancelled. If you realize that you missed the 48 hour period (eg, it is 24 hours before the appointment), you can call to see if we have not filled the time with someone else.           

 

In summary, there are two types of paperwork you will do. One is paperwork you print from the website (fill it out and bring it with you); the second is paperwork you do "online".
